The survey will not take long to complete and everything you tell us is anonymous and confidential. We understand that some of the questions may be sensitive and appreciate your open and honest responses so that we can help more people in Erewash to move.

As a thank you for taking part in the survey, there will be a random prize draw for 5 winners to receive a £40 gift bag, helping them to be active at home.
